This is going to be pretty raw:

Marriot Sky Tower I-57 and 103rd (Olive-Harve Community college?)

Elemental Hall (not damaged or endangered) sits in the lake at
about 55th (Garfield) behind the U of Chgo.

Harold Washington Community Hospital -- No address given but
Ill make a guess that it sits on the site of, or near
where Loop college (now called harold washington) is.

Handelman Institute -- 2 miles north of Truman Towers.

Special Ops FIRST Jump site -- North and Western aves.

Harlem and Irving Park (Aprox) first attack site
(OR -- Harlem NORTH of Irving Park). They give both locations
during the description.

Universal Brotherhood Merchandising -- Madison and Sangamon
[I guess this is where the main hive was]
