It's rather an, er, _interesting_ idea in all. As far as I can see, the
way it works is that if you modify the "you" that came about throught the
natural span of time, you lose essence (and magic, if a mage). So if you meddle
with Mother Nature (always capitalize that-- don't like what happens when I
don't), you pay for it in a very tangible way. This isn't the result of
training, or any such "normal" process, but of direct intervention.

As far as the essence goes, I would say "to heck with it. If you spend
enough money on a doc, cultured ware, and enough Karma on re-rolls and autos,
why pay?" For game purposes, it would be interesting to see what a mage who
was modified like this would be like. Going to have difficulty convincing my
GM, though.
