Actually, a good way to look at this is something like "negative
reinforcement." If a character has a "good" moral code that (s)he lives
by, then shooting an innocent can associate negative connotations to the
use of that skill and thus that character does not improve that skill for
that instant. A "good" moral code means killing innocents is against what
you stand for.
This, of course, is moot if the character is a cold blooded killer.
