on shadowtalk how about this modifier.
Change the duration of the spell to sustained with a minimum duration of 20
turns. Essence is drained by 1 point or if the charcater only has 1 or less
points of essence then it is drained so that resultant essence is 0.001 (or
some other small figure). The point of essence is only lost after the 20
turns has passed after that the spell can be sustained further just to cause
more pain.
When the spell is dropped the character's essence goes back to normal, though
the character still undergoes the maddening rage. The only way to make the
essence loss permanent is by killing the character while the spell is being
sustained on him, in which case it matters little as the character is already
dead.
I also agree with Todd's (I think) idea about making it the domain of NPC's
by some overriding influence such as a toxic shaman. However as I orginally
designed it for a mage, how about making it a condition that any character
that has this has to be certifiably mad, hence out of players control.
