Could you make a spell that needed to be sustained exclusive? In other words
while you were casting the spell you are not sustaining anything. And then
when it is finished being cast sustain it? Anybody got any ideas?

You certainl can... the catch is that it is still exclusive while you are
sustaining it. So you can't cast any other spells, or perform other magical
activities, while you sustain the exclusive spell.

I'm not sure I understand the question.. The SR3 definition of an exclusive
spell is one that can't be cast or sustained while another spell is being
sustained. If you have two spells that are NOT exclusive, you can cast one
while sustaining the other. If one of the spells is exclusive, you can't do
this. Any spell can be made exclusive; you decide this when you learn the
spell.

If that doesn't answer the question, could you rephrase it? Or give a more
detailed example?

COuldn't have said it better myself...

However, I was re-reading the rules, and it sounds like you cannot stack
fetish and exculsive restrictions on a spell. It's not all that clear in how
they word it, but if I infer correctly, you cannot use both to boost a spell 3
force categories. Anyone care to back me up or disupte this?

If I understand you, you are correct.

Page 181, BABY: "A spell can have both a fetish and exclusive modifier,
but they must be seperate in purpose."

In other words, each can either reduce the karma cost to learn the spell
-OR- reduce the force of the spell for drain. When you take both the
fetish and exclusive spell limitations, they must be applied towards
different reductions.
