How about shooting someone with a powerful handgun and having go thru the
target thru the wall into the guy next door.

Use expanding ammo, and make the powder load a little lighter. I'm serious.
That's one of the reasons I hate jacketed slugs and hot loads; they increase
the chance of doing just that. And in a real-world situation, that might
not be the guy next door; it might be your kid in the next room.

Hmmm...safety rounds in Shadowrun. COnsidering the thin walls in most
slums, I think maybe those might be a good idea. Safety rounds also tend
to do a lot more damage to unarmored humans, I would think...something
about soft lead flattening out when it hits bone or something just seems
to shout "Massive internal trauma" to me...

"Safety" rounds are usually designed to fragment when they hit something
hard, like a wall (or a body part). That does tend to cause more damage
than regular ammo, IIRC, but a small problem is that it's impossible to
make rounds that break up against walls and have good armor piercing
capabilities (obviously), thus anyone wearing body armor will be much
safer from this kind of ammo.

In SR, such ammo should face double or triple Barrier Ratings at least for
getting through the barrier, and do the same with armor ratings. Then give
it something like +1 Damage Level vs. unarmored targets, and you have
something close to SR's flechette rounds :)
