Hmmm ... interesting topic.
I've never actually killed a PLAYER per se. I have made a few wish they were
dead.
I've always felt the best way to do it would be to drive them to geek
themselves by continually frustrating their characters every plans and
putting them in miserable soul-numbing situations.

You know kill their whole family on Christmas Eve, cook their dog and serve
it to them - that sort of thing.
I could tell you more, but I don't want to be responsible for someone
actually using these ideas! ;-)
