Okay, could someone fill me in? What exactly is "my bad"? I've seen
it enough that it can't be just a grammatical error...what the heck am

It's slang. It means 'my mistake'.

For some reason, a lot of slang seems to be a regression of speech back to
babyhood.

Depends on the language. I think most russian slang was conceived in a
fit of drunken anger, as the words just sound like they're being
pronounced by a drunk person in a hurry to expel as many imprecations per
unit time as possible... :^)

Surely it depends on the languange :-)
Greek slang for instance tends to abuse every single grammar rule mixed
with lots of swearing and foreign words :-)
