OK, here goes. The e-mail addresses you're most likely to get
some sort of response from are:

FASAMike@***.com (Mike Mulvihill) and...
FASALou@***.com (Lou Prosperi)

While they actually *do* check these accounts, be aware that they
are busy and it may be days, or even weeks until you get a response. It
will eventually come, though.
The business address that you need to send your
Self-Addressed-Stamped-Envelope to is:

FASA Corporation
1100 W. Cermak B 305
Chicago, IL 60608

I can't remember if there's a specific department to Attn: to, so you
may want to ask. I hope you have better luck getting a response than I
did. If you do, photocopy the stuff and pass it my way :)
