Part of the SINless population has entered the country illegally, I think.
For example people from the NAN who came over to Seattle to make a name
for themselves (or just get a better life) in the big city, but ended up
at the bottom of the ladder instead -- just look at places like Mexico
City for plenty of examples of today's equivalent.

As for people not born in a hospital, I'd say there are a lot of them as
well. Hospitals charge money for their services, so if you don't have that
you won't get in. Those with a SIN and a job at some or another corp
either get to go to the corp's own hospitals (at a much reduced rate than
the "public" hospitals, I would guess) and/or have health insurance that
pays the hospital bill for them for necessary stays there.

Someone living in the Redmond Barrens would not, I think, qualify for
either of those ways of paying for the bill...

I guess that's where folks like the Yakuza come in, the 'interested in their
protectorate' organized criminals. Yeah, I can see that. Yakuza find some
skilled-but-not licensed medico to take care of such things, provide minimal
medical care and supervision for the delivery, and midwives... Wow, back to
the middle ages, no one but the rich get medicine. Between that and the
'icon-literacy' that most non-corps get, SR *is* set in the middle ages, with
better swords and communication.
