Here is something that I created recently in an effort to try to get some
feedback and suggestions from my players. I would like some comments on the
questions asked, suitability of the questions, other things that should be
added, and your general opinion on the idea. Thanks guys.

Shadowrun GM Survey Form

Please answer the following questions by circling the appropriate rating. 1
being "suckworthy", and 5 being "godlike". Do not be concerned about
insulting or criticising your GM, as receiving feedback is the only way a GM
will improve. Please be honest.

Did you enjoy the game overall? 1 2 3 4 5
NPC characterisation: 1 2 3 4 5
(How well and realistically did the GM portray NPCS? How much variety did
PCs personalities have?)
Plotline: 1 2 3 4 5
(Did you like the storyline? Too small? Too epic? Too convoluted? Too
simple?)
GM storytelling: 1 2 3 4 5
(How well can your GM spin a tale? Do you get bored easily? Does the GM
keep things going and things happening?)
Descriptions: 1 2 3 4 5
(How well does your GM describe places, people and actions? Not enough?
Way too much (snooze...)?)
Amount of combat: 1 2 3 4 5
(Too much killing things? Not enough? We the things killed too wussy? Too
hard?)
Amount of planning/problem solving: 1 2 3 4 5
(Does the problem solving and scheming annoy you? Do you enjoy it? Was
there too much or too little? Was it too hard or too easy?)
Amount of roleplaying: 1 2 3 4 5
(Did you think that roleplaying just didn't happen? Was it low quality when
it did? Was there too much character interaction?)
Dice rolling: 1 2 3 4 5
(Did too many dice get rolled? Not enough dice? Were dice rolled for
things that mattered?)

Damion, I wouldn't use this on your players after every run. That would be
going too far. However, it would be good to do for a while...to get the feel of
how your players want the campaign to go. And then once in a good long
while, to get the feel of any changes the players have gone through.

Otherwise, it's pretty cool.

Damion Milliken brought us his survey form.
Quite good, but I don't see the point. I keep a GM diary. After each
session it's critics time, and my players tell me what the liked and
disliked. I write it down and try to make it better. IMHO there's no
need for any form to be filled out, we have enough paperwork, ain't
we.

We do a similar thing after our game sessios. While everyone is winding
down, and chatting about things to do and things done, I go orf and make
coffee, and we all usually have a chat about things we did or didn't
like, it just sort of naturally goes that way, and seemed to stick. I
get to tell the players where I feel they went wrong, and played out of
character, and they get to tell me where they feel I was either too
lenient ar too tough. It works for us, don't need no survey. :)

We don't exactly keep a diary, but we do keep track of the dates
of the run and which characters were on them. Since we have
several characters each (we *usually* only play one character at
a time) it allows us to multi-task our games. The number of players
at our sessions, which are held twice a week, fluctuates between
four and eight. We have been known to have seven runs on the go
at once, spread over a six month period, and on four different
continents. Thinking about it make your brain hurt, I can tell you!
