The next 2 points up to the maximum of skill, cost 2
skill points each. That's another 4 points, for a
total cost of 8.

In tabular form:
Skill - 6
Linked Attribute - 4
Skill Level Points/Level Total Points Cost
1 1 1
2 1 2
3 1 3
4 1 4
5 2 6
6 2 8

Clear enough for ya? ;-)

AFAIK (I can't say definitively, as I've never had one
stuck in me), a shiv is a makeshift knife usually
found in jails. It can be made out of practically
anything, so long as it's got a blade (usually
sharpened on the concrete floors) and a handle.

I believe they're also called "shanks" and probably
have myriad other names, depending on where in the
world you find it (sticking out of you).

Lovely things. Even if it doesn't kill you, you're
probably going to get infected but good.

*Doc' had a special knife, once. It was called a
K'niffy and it was magical.

How is it that Captain Chaos manages to keep the ot stuff to a minimum?
I mean the most OT I've ever seen was in cyberpirates.

The GM and decker player have to KNOW the rules well.

All the tests to be made are basically in the same format and can be
handled very quickly if the participants know exactly what to do and
get it over with ASAP. I've found that few tests are really essential.
The Logon to Host operation is indespensible and from there its up to
the decker as to how many tests he makes. Remember, the more things he
wants to do the higher his Security Tally is going to go and the
sooner he is going to be forced to leave or fight

A "shim" is a tapering piece of material (often wood) used when leveling
objects (such as a door frame during construction). "Shiv" has been
correctly defined several times already.

Actually, the following from the SR3 book says otherwise: (pg 57, left column paragraph 6)
The
last sentence of the paragraph reads: "For skills with ratings higher than the linked
Attribute, each
point over the Attribute Rating costs 2 Skill Points."

What am I hoping for? Ideally? Something like Danila (I think I'm
mixing up her and Danilo. I'm thinking of Cadderly's wife).

Strong, quick, agile, doesn't need armor or weapons, but can be well-trained
in them. Can meditate for (minorly) increased hp regeneration, can resist
mental/emotional attacks better than most.
