Does anyone have any approximate costs for surgery to implant cyberware,
I remember that it was covered in SR1, the SR3 doesn't seem to mention
it and I would like to know if possible.

And just to clarify I mean surgery costs to implant cyberware after
character creation.

These costs were in SR1, as you remember, and also in Cybertechnology.
They were not changed from those in SR1, so if you have access to that, I
suggest you simply keep that book (or at least the surgery costs) at hand
when playing.

Collectively, these are known as "elective surgery"- thats the section
heading, but it never appears in any index or table of contents. (GRRR).
Hence my complete posting here.

The relevant pages are;
sr2 p114
Shadowtech p 6
Cybertechnology p 53

Although the costs are listed in Cybertechnology, the meaning of the
terms are not: those are in SR2, or Shadowtech, if your looking to implant
bioware. Neither ever lists costs.
Man and Machine will have all these figures in one place (if it
doesn't- well, I do live in Chicago...). The figures given are, IMO,
somewhat impractical in game play, so I'd expect them to change in Man and
Machine (SOTA, doncha know), which should generally expand surgical
options anyhow.

Heres the compilation of relevant figures (FAQ me while you can):

Minor Cosmetic-
Inflicts a light wound. Items of no essence cost, most current day
plastic surgery. (sr2)
BI .25 or less (ST)
1000 ¥ (CT)

Minor invasive
Causes moderate wound. Items up to .4 essence, whole skin grafts, major
cosmetic modification involving muscles or nerves. Example given of a
risible head crest! (sr2) [note- how this differs from some bioware, I
don't know]
.26 - .75 BI (ST)
5000¥ (CT)

Major invasive
Serious wound. .41 to .99 essence, organic limb replacement, eye
transplants (useless until fully healed). (sr2)
.76 to 1.5 BI (S.T.)
25,000 ¥ (CT)

Drastic invasive
Causes deadly wound, but patient is stable and won't die. 1 or More
essence, all organ transplants. Can cause magic loss (check as normal
deadly wound). (sr2)
More than 1.5 BI, >>>All Nueralware<<< (ST, emphasis mine)
>>>250,000¥<<< (CT, emphasis mine)

Why the emphasis? Well, in my experience, this basically means you
don't get nueralware after character creation, as the surgery cost much
more than the bioware itself.
Another note; it sometimes pays to get custom chrome and a good
surgeon, since you can get the essence cost down to the point where its
cheaper surgery (unless you also assume the surgery costs are at black
clinic rates, in which case you'll never get any big pieces of custom
chrome).
I presume that each item implanted is usually done separately: its
almost always cheaper, and total healing time is usually less. If for
some reason you wanted a bunch of little things in one whak, I suppose you
could pay for the Drastic surgery, though.
