Sounds like a case for new players. I could accept a reply along the
lines of 'go to hell' though characters prepared to face certian
death to avoid doing something that does not guarantee them death or
anoything worse should be pretty rare really.

I generally recon that 1/2 karma is a fair penalty for stupidity. Get
your character killed and the next comes back at 1/2 your old total,
at a suitable point. If it wasn't the players fault i am usually more
lenient and if a new player joins a reasonably powered game i usually
hand out something around 90% of the lowest characters karma total
(as a fair range of karma totals due to varying attendance and
roleplaying ability usually appears given time)
All this can be further complicated though by the dreaded 'real world
political situations' magicans joing high powered games where mundane
replacement characters exist at low totals (having been 1/2 karmered,
the mage needs more karma to be fair than a mundane while the mundane
usually needs more money) etc.
