I wouldn't allow it (especially after the Goro reference :) ). Basically,
the motor centers of the human (or metahuman) brain don't have the wiring
to handle extra limbs. The sensory inputs alone occupy a big chunk of the
brain stem (I have some really spiffy maps). Then you get to muscle control,
reflex handling, and conscious motor control and it gets tight in there.
Sometime, try typing with both hands and tapping out the rhythm for "Dixie"
with your feet while carrying on a conversation with two different people.
Your brain just isn't wired for it.

Then there's the center of gravity and balance issues, raw physical issues
like where to build in the joint (there's no support structure in the mid
torso to hang a limb on), strain on internal organs from compression and
torsion ...

I wouldn't allow it in my campaign.
