The highest I've seen is 18+4D6, and that was in the speed sam of our own
design. He was a joke character, created when we got talking about how
high you can get your initiative for a staring character, to the expense
of everything else. Throwing in Reaction Enhancers, from Cybertech (which
wasn't out when we made the speed sam), you should be able to go higher
-- 21 Reaction is possible, by taking only Wired Refs level 3 and 3
Reaction Enhancers (total: 5.9 Essence) for cyberware.

I'm not surprised. It might be a great char for a one-off adventure,
though.
