SRII, page 72, simply says Etiqutte requires you take a Concetration
at character creation. It doesn't say this is handled any differently
than concentrating or specializing in any other skill. Thus my
understanding is that 5 point put in Etiquette for a new character
could mean you'd have something like Etiquette 4 and a Street
concentration at 6. If you chose to specialize, the same 5 points
could come out Etiquette 3, Street 5, The Ancients 7.

SRII, page 191: "New Concentrations are based on the existing general
skill score. If the character has Firearms 4 and wants to concentrate
with Pistols at 5, the Concetration would cost 5 x 1.5, or 8 Karma
Points."

Also: "New Specializations are based on the existing Concetration
score. If the character does not have an appropriate Concentration,
use the general skill. Thus if the character only want to improve in
the Remington Roomsweeper, the Specialization would cost 5 x 1, or 5
Karma Points."

The above means two things.

First, common sense should suggest that you'd have to have a base
skill of Etiquette, otherwise what would you be figuring the cost to
start a new concentration from?

Second, to answer your above question. If the character that had
Etiquette 4 and Street 6 wanted to concentrate in Mafia, it would cost
him 8 karma to start it at 5 (1 point higher than the base skill at 4).
