Precisely. This isn't AD&D where the target stands more or less still
waiting for you to make an attack that either bounces off the target's
armor, or finds an opening in it.

Those would not get to use their Combat Pool.

Ditto.

Again, the same as above.

They could use the rifle to block, or as a melee weapon. (Note: don't do
it as in the TV series Sharpe *grumble &%^&# Net 5 grumble &#%^&*$
Wimbledon grumble* or you'll end up with half a rifle pretty soon.)

SR melee combat _is_ vague; most attempts to make it less so only
complicate matters so much that it's easier to go back to the vague system
than to use the more accurate one... The way to penalize charactrs who are
not "on guard" is to deny them their Combat Pool, as that way they'll
often have a lot less dice to roll and so will be easier to hit.
